Our full assessment and placement process can begin with our online senior assessment form. After we review your completed form, we will contact you for an in-depth discussion — by phone or in person. Multiple hours of face-to-face, phone, and email consultations are free if moving within 90 days. Future placement planning and other eldercare planning, and crisis handling, monitoring,and reporting are offered at $195 per hour.

A care plan tailored your specific circumstances is prepared and is continuously monitored and modified as needed. This process may take more than one visit with the patient and family members. The comprehensive assessment is a compilation of smaller individual assessments with the first one being a primary intake assessment which includes demographic type data as well as a health history, social history, & legal/financial history. A medication profile assessment is established.

Next we do an in-person home safety assessment, especially balance and fall risk assessments, nutritional assessment, assessment of any pain or depression, a cognitive assessment, and we explain options for eligibility for government assistance.

We have extensive knowledge about the costs, quality, and availability of services throughout the Greater metro-Sacramento region as well as Placer, Nevada, El Dorado, Yolo, & Yuba County cities and communities.

Required forms can be completed and are valid for up to 6 months. Admission Requirements

Physician’s Report forms are required for most residential communities and are a helpful tool if the senior is not available for personal interviews. There is a standardized physician’s form that can be obtained from licensing offices, hospitals, and residential communities. These forms must be signed by the senior or by the conservator or the person with Durable Power of Attorney.

Generally, it is best to have the physician’s portion of the form completed by the senior’s primary physician. A recent TB clearance report is required. Once signed and dated by the physician the form is valid for 6 months.

Acceptance into a residential community requires a pre-appraisal interview of the senior by a representative from the community (except for out of state applicants). The facility has the right to refuse a resident at the time of a pre-admission interview. An assessment and referral to an appropriate community by Senior Care of Sacramento minimizes the chance of rejection occurring.

Residential facilities may require a primary responsible party for fiduciary and health actions. These might include Power of Attorney for Medical and Financial concerns (including Living Wills and/or Advanced Directives), arrangements with the senior’s bank, a personal lawyer or a written acceptance of financial responsibility by a family member.

Most residential facilities have admission agreements (simple contracts). The senior or family member should request a copy and, prior to signing, review it with the appropriate person at the facility. If there are specific concerns the Registry is available for consultation.

Financial Realities

Average cost for an ambulatory semi-private accommodation (sharing a room with one other person) residential care community in the Sacramento region is $2000 per month. Non-ambulatory, semiprivate average $2500. Private accommodations average $3000 for ambulatory and $3500 for non-ambulatory. Independent Living studios start around $2500.

These are basic charges and are subject to increase depending on the amount of care the senior requires such as lifting, incontinence, and/or dementia. Charges may also decrease when the senior’s needed level of care decreases.

The majority of facilities have a fee structure similar to apartment rental. There is a monthly rental fee to be paid prior to move in with the requirement that 30 days written notice of vacating be given.

Some communities require an entry fee, some an application fee, and some require first and last month rent upon entry. Usually the fee covers all the basic living expenses including furnishings (unless the elder wishes to furnish their living accommodations), with extra fees for a private phone, long distance charges, cable TV, Internet access, laundry and dry cleaning services, incontinence supplies, medication costs, beauty supplies and other personal supplies.

Medicare and Medi-Cal do not pay for any of the above costs because these communities are classified as non-medical or offering custodial care. Some long-term care insurance does include residential living.

Supplemental Security Income (SSI) may be available for some seniors with limited income. Visit your local Social Security Administration office for this information. Only a limited number of communities will accept SSI as the full monthly payment. Generally, those accepting SSI as the full payment require that the senior be ambulatory and require minimal care. SSI benefits can be supplemented by family or friends to cover care beyond basic services without any penalties or cuts in the senior’s monthly benefits.
